SUCCESSFUL BARIATRIC SURGERY WAS CARRIED OUT FOR THE FIRST TIME IN UNI HOSPITAL

A 56-year-old patient from England successfully underwent revision laparoscopic bariatric surgery. A similar type of intervention was performed for the first time at Uni Hospital and the region by Dr. Ivaylo Tsvetkov. Dr. Tsvetkov is a consultant in bariatric and metabolic surgery at North Midlands University Hospital, Stoke-on-Trent, UK. He explained that he made a new modification of the patient’s existing gastric bypass with the aim of a long-term weight reduction  returned after the first operation.

„The operation went normally, the patient was mobilized immediately and discharged on the fourth day,“ explained Dr. Tsvetkov, who has performed nearly 900 weight-loss operations. The patient underwent laparoscopic gastric bypass in England in 2009. With this operation, he successfully reduced a large part of his extra wight and significantly improved his quality of life, but 12 years later, for a number of reasons, he regained more than 50% of the lost weight. There are other health problems that require – hip surgery and treatment for Type 2 Diabetes. The consultations with doctors in England and their recommendations are to lose at least 40 kg again. Due to the long waiting period for such an operation in UK and after conducting research, the patient from England decided to undergo the operation in Bulgaria, at „Uni Hospital“, Panagyurishte. His selection was based on references from other English patients regarding Dr. Tsvetkov’s qualifications, as well as the hospital’s acquired certificates and license to treat patients from abroad.

It has been proven that already on the tenth day after gastric bypass, the patient’s blood sugar normalizes, 2 to 3 months after the operation, sleep apnea disappears, hypertension and cholesterol normalize, joint and back pain subsides,  metabolic syndrome improves, women’s chances of becoming pregnant increase, the risk of breast, ovarian, colon cancer decreases, and most importantly: the quality of life of the operated patients significantly improves,“ said Dr. Tsvetkov.

The 130th anniversary of the establishment of the first medical institution in Panagyurishte – a hospital with 20 beds and 6 years since the opening of Uni Hospital, the legal successor of Sabo Nikolov Hospital, was marked with a water dedication for health and a presentation forum. The Attorney General of the Panagyurishte Spiritual District, Atanas Manolov, prayed for health and wished well-being to the entire team of the hospital in a solemn ritual attended by the mayor of Panagyurishte, the management of the hospital, representatives of the Bulgarian Medical Union, prominent medics, etc.

Angel Prodanov, procurator and Medical director at the hospital presented its history, emphasizing that today there would be no holiday if it were not for the work of Prof. Lachezar Tsotsorkov – initiator and engine for implementation of the project for the establishment of Uni Hospital. „Prof. Tsotsorkov was a genuine visionary, he foresaw the future and the place in it of a modern medical institution, he managed to unite huge public support and civic trust for this cause,“ Dr. Prodanov said.

The manager of the hospital Dr. Yavor Drenski stated at the beginning of his presentation that the hospital is an example not only in the field of medicine, but also a model of how public-private partnership between Panagyurishte Municipality and Assarel-Medet Ltd. can be successful and to benefit the community and patients. Dr. Drenski pointed out that since 2016 more than 97,000 patients have been treated at the hospital, more than 16,000 operations have been performed, and more than 1,000 deliveries were conducted. IAVOR DRENSKI, MD: DRG LEADS TO FAIR HEALTHCARE FUNDING AND TO IMPROVING QUALITY IN HOSPITALS

The system is a tool to support budgetary discipline in healthcare, Dr. Drenski stated at Kapital Digital Health Forum

The system of diagnostic-related groups (DRG) as a mechanism for financing hospital care allows a fairer allocation of funds and helps to improve the quality of activities. This was said by Dr. Iavor Drenski, MD, Manager of Uni Hospital, at the conference Digital Health Forum, organized by Kapital and HealthPR on May 17, 2022. Dr. Drenski took part in the panel „Application of Digitalization for the benefit of patients and healthcare facilities“ with a speech on effective and well-established methods for financing and control of costs and quality in hospital care.

„One of the most complex processes in the economy is to evaluate the hospital service due to the uniqueness of each patient,“ explained Dr. Drenski. The perfect method has not yet been discovered, but in the world practice the financing through DRG is confirmed as the most appropriate. In Bulgaria, its introduction has been underway since 1997, but clinical pathways have been used as a funding mechanism up to date. They were created to maintain the quality of medical institutions, but in Bulgaria they were adapted as a system for hospital care reimbursement, which had to be temporary, Dr. Drenski recalled. Another major weakness of clinical pathways is that they require very complex and not very accurate reporting.

Diagnostically-related groups combine similar diseases or procedures for which a similar resource is spent. They are a powerful tool and a guarantee for effective cost control. Their specificity consists in the relative weights – they show the ratio between the various diseases in relation to the resources and the volume of activities that need to be performed. „Through DRG we will have a much fairer distribution of resources between individual diseases and procedures. In this way we can build a case-mix index of hospitals – we will know which treat severe cases and which are structural determinants of the health system“, said Dr. Drenski. Thanks to the case mix of hospitals, patients can be referred to those medical facilities for which the highest indicators and results in the treatment of individual diseases have been documented.

Another positive is that DRGs require a transparent accountability system and provide a tool for controlling the healthcare budget. It is applied according to a formula in which the basic value is essential – what is the public budgetary framework for health care for the year. „The baseline can be updated to maintain the ratio between individual DRGs, while paying for hospital care remains within the budget,“ said Dr. Drenski.

DRGs are also a tool for quality control and improvement, due to the fact that in this system the doctor sees in the patient not a clinical path, as it is now, but a person with a health issue and all his needs. The doctor’s goal is to cover the entire process of medical care, treating not only the major disease but also the comorbidities. The more multidisciplinary the patient is treated, the higher is the level of reimbursement. DRGs report not only the volume of activity performed, but also assess the age, accompanying conditions, complications, etc.

The system of diagnostic-related groups allows quality control of the hospital outcome, that is not applied in our country yet. It is also a specific and reliable tool for in-hospital management, as it is easy to find problematic structures, activities that generate more costs than expected for a given DRG. Benchmarking between hospitals can also be carried out and thus the hospital management can discover where the facility is located in terms of cost and quality indicators

Traditionally, the new Govermor was represented by Dipl.Eng. From 2010 to 2016, Dr. Drenski was CEO of Tokuda Hospital Sofia. He has been part of the Japanese hospital team since its inception, successively holding the positions of Administrative Director (2005-2009) and Medical Director (2009-2010). In the period 2002 – 2005 he was „Hospital care“ Director at the National Health Insurance Fund. Dr. Drenski has extensive experience as an international health consultant to global companies such as Bearing Point, 3M, Deloitte Touche Tohmatsu, etc.

Dr. Yavor Drenski graduated Medicine in 1992 at Medical University – Sofia. His medical education continues with a specialization in Obstetrics and Gynecology, working for 8 years as a physician. He holds Master’s degree in Healthcare Management since 2009, and Master’s degree in Finance since 2003. He has completed a number of specializations in hospital management and hospital finance – in the United States, Switzerland, Hungary, Britain, Japan and others. .

In 2013 he was one of the founders and first Chairman of the National Association of Private Hospitals. In 2016 he was Chairman of the Board of the Bulgarian Association for Personalized Medicine, and later Executive Director, as he is now.

This was stated by President Rumen Radev at the ceremony of awarding the three outstanding scientists with high honors.

For her contribution to the development of the medical education, President Radev awarded Prof. Nevena Tsacheva with the Order of „St. St. Cyril and Methodius” second degree. Prof. Tsacheva is a Head of the Occupational Medicine Service at the multidisciplinary hospital „Uni Hospital“. She is the founder and first head of the only European comparable department of Occupational Medicine at the Medical University in Sofia. Prof. Tsacheva created the first educational-qualification degrees „Bachelor“ and „Master“ with 19 study programs in „Occupational Medicine and Work Ability“. Prof. Nevena Tsacheva creates and manages the National Register of Occupational Diseases as an external body of the National Statistical Institute, she creates a Unified Health Integrated File with a practical methodology for assessment and analysis of the health of insured persons.

The volleyball tournament „He is“ in support of men with prostate cancer took place on October 3 at the “Arena Asarel” sports hall. The tournament is the third event of the „Winners“ initiative of the Oncology Center at Uni Hospital.

Dr. Rossitza Krasteva, Head of the Center and initiator of the campaign, said that its purpose is to remind that accurate and timely diagnosis and good treatment can turn patients to winners without drastically changing their style and quality of life.

Intense and filled with tense situations and joyful emotions, the volleyball tournament „He is“ became an unforgettable event for all participants – it gathered doctors, patients, their relatives and friends.

At the end of the tournament awards were attributed to the winning teams and the most outstanding players.

The team of the „The Saints“ from Berkovitsa with captain Petyo Petkov, who had demonstrated enviable skills on the field, ranked first. The second place went to the veterans of „Pan-Volley“ with captain Dobrin Kolev, and the third place went to the young athletes of „Pan-Volley“ with captain Kristian Tilovski. The team of „Calypso“, led by the former national volleyball player and current coach Naiden Naidenov, was fourth, and the team included doctors from the Oncology Center at Uni Hospital – Dr. Zahari Zahariev, Dr. Tsvetan Tatarov, Dr. Bozhidar Iliev, Dr. Khalil Ashimov. The fifth place went to the team of „The Experts“ with captain Dr. Krassimir Shopov, chairman of the „Experts Against Cancer“ Foundation, and the game of Nikolai Gospodinov, deputy manager of Uni Hospital, also impressed the participants and guests.

Dr. Mihail Iliev, a longtime doctor of the football team PFC Levski (Sofia) and of the national team of Bulgaria, had prepared special awards for the most outstanding participants.

 

The maintenance of constant physical activity is of particular importance for self-confidence and good health, said Dr. Krasteva and commented on possible sports activities depending on the physical condition of patients.

PCR TESTS ARE ALSO PERFORMED ON SATURDAYS AT UNI HOSPITAL

The PCR test is defined as the gold standard for the detection of COVID-19.

Due to great interest in the test for detection of SARS-COV-2 (COVID-19) infection by PCR-based method performed in Uni Hospital, samples are collected every working day from 8 am to 3 pm. On Saturdays, samples are taken from 8 am to 10 am. For those who travel abroad, the medical facility continues to issue a Certificate in English.

The tests are performed by the Microbiology Laboratory, which is the only laboratory in the Pazardzhik region approved by the Ministry of Health for the identification of coronavirus infection. The PCR-based method is defined as the gold standard for the detection of COVID-19.

Now is the moment for all Bulgarians to be united, to follow the rules approved by the experts in the Government and the World Health Organization, because only together will we succeed.” This was stated by the chairman of the Foundation and the Supervisory Board of Assarel-Medet, Dimitar Tsotsorkov. Mr. Tsotsorkov underlined that he and his team remain faithful to the motto „Going first means going on time“ and added: „The foundation and the companies I manage, want to help the people and institutions that are and will continue to be at the forefront of the fight against the pandemic in Bulgaria.“

In order to meet the medical teams’ needs, Lachezar Tsotsorkov Foundation and Uni Hospital will donate 1,000 face shields, 300 suits with silver-ion protection, 7,000 reusable masks, 1,000 liters of disinfectant, as well as 10,000 express testing kits for screening purposes. 5,000 of these express tests will be donated directly to the Ministry of Health of the Republic of Bulgaria and the National Operational Headquarters for fight with the Pandemic.

The protective equipment and the remaining 5,000 tests will be donated to medical facilities that meet the wave of complicated cases of COVID-19 patients and are under a higher risk of increasing number of hospitalized Covid-19 patients in deteriorating condition. A non-exhaustive list of the healthcare facilities the help will be directed to: University Hospital „Pirogov“, University Hospital „Alexandrovska“, University Hospital „St. Anna“, University Hospital „St. George“, Multidisciplinary Hospital for active treatment „ISUL“, Multidisciplinary Hospital for active treatment „Pazardzhik“, Multidisciplinary Hospital for active treatment „Dobrich”, Specialized Hospital in obstetrics and gynecology „Maichin Dom“, University Hospital „St. Marina“ and others.
